Nomination/Acceptance Forms Checklist

Congratulations again on your placement with the National Student Exchange! You'll be receiving a packet of informational materials at the upcoming NSE Orientation. In addition, please print, complete, and return the following to Study Abroad and Exchange Programs.

Conditions of Participation & Program Acceptance Form
Medical Release and Medical Report Forms: If your host universityrequires you to complete its own medical forms, please send us a copy.


Other things to do:

In addition to completing these forms, there are several other things you'll need to do before leaving:

  • Attend pre-departure orientation - Orientation dates and details were included in the e-mail sent to you with your placement information. If you are unable to attend, you must contact your advisor no less than one week in advance to schedule an alternative time.
  • Pay any fees/deposits -
    • If you're on Plan A, you may be responsible for fees or deposits at your host university. Please check the materials from your host university when they arrive, or contact the host NSE coordinator.
    • If you're on plan Plan B, you must pay a $300 deposit to CWU upon acceptance to the program.
    • For further explanation of program-specific fees, please see the What Does it Cost? page.
  • Credit Transfer Agreement (optional) - Completing this form allows you to substitute classes abroad for CWU's major, minor, and general education requirements (subject to departmental approval). This form can be completed before or after your exchange, but for academic planning purposes it is helpful to complete it in advance. Please see the form for detailed instructions.
  • Review your program dates - You should soon be receiving information from your host university. Check with the sponsor of your program and review your formal acceptance materials to confirm the start and end dates.
  • Make your travel arrangements
  • Research vaccinations and health needs - For traveler's health information, including vaccinations, please visit http://www.cdc.gov/travel/. We also recommend that students with preexisting conditions, prescriptions, or special needs consult with their physicians, site directors, and/or advisors about how to accommodate these needs while away.
